The War Memorial Plaque that was originally in the Venerable Bede Church, Gateshead, was transferred to two other locations in succession before finally being placed in St. Edmund's Chapel (241 High Street, Gateshead).

The memorial is made of three plaques. The top one is nearly 10 feet (3 m), long and shallow, with the top corners rounded inwards, with an engraved narrow border, black-edged. This bears the dedication in incised capitals painted gold. The other two plaques also have narrow engraved borders, and on each the names are listed in four columns, the letters incised and gilded using sans serif capitals. The plaques are made of grey marble.

The North East War Memorials Project has provided a transcription of these names.

Lance Corporal John Thomas Burns. 16932. 6th Battalion the Yorkshire Regiment. Killed 22 August 1915.
Born Gateshead, Enlisted Newcastle, Resided Gateshead.
Commemorated on Panels 55 to 58, HELLES MEMORIAL.

Private Charles Flintoff. 1724. 4th Battalion Yorkshire Regiment. Son of Mrs. Caroline King (formerly Flintoff), of 43, Tennyson St., Gateshead. Killed 25 April 1915. Aged 19.
Born Newcastle, Enlisted Middlesbrough, Resided Gateshead.
Commemorated on Panel 33, YPRES (MENIN GATE) MEMORIAL.

Corporal John William Miller, 16894. 6th Battalion Yorkshire Regiment. Killed 22 August 1915.
Born Gateshead, Enlisted Newcastle, Resided Gateshead.

Private George Arthur Wrightson, 46588. 2nd Battalion the Yorkshire Regiment. Son of Thomas and Jane Ann Wrightson; husband of Isabella Mary Wrightson, of 98, Nile St., Gateshead, Co. Durham. Killed 22 March 1918. Aged 27.
Born Gateshead, Enlisted Gateshead.
